Monthly Contact Form

The purpose of the Monthly Contact Report is to maintain contact with our adopted families and to provide Child Safe with updates as required. Liaisons are required to contact their adopted family/families on a monthly basis. Families must be contacted prior to the 15th of the month. You are required to submit the Monthly Contact Report to the GOC Liaison Coordinator by the 3rd Tuesday of the month for our monthly GOC Chapter meeting.

Patched Members may only visit with their LG when accompanied by another Member of GOC; the parent or legal guardian must be present. The parent/legal guardian must provide approval prior to any contact with the LG. Our presence will be available as long as the child needs us.
